A seat belt cutter or knife is the one small item I would suggest people have to self extricate in the event of a crash. Another possible item to have is a window punch or other item to break glass. It's very difficult to break glass with your bare hands and in the event of a catastrophic crash, the electronics may fail, leaving you trapped in a burning or submerged car. But my contention has always been that the stuff you find in a car first aid kit is near useless in a crash unless all you need is a band aid for your little boo boo. The real life saving stuff-- something to restore positive airway, control major bleeding, to address major trauma to tissue, and to stabilize fractures don't come in a car first aid kit. For that sort of thing, you need proper training, and equipment.
